a betta fish it will help him gain a sense of himself if you help him feed it and take care of it and have him give it his own special name. works every time

I suggest that you take him to give his old toys away to charity on his birthday and then let him pick out a new bike. He'll learn that giving to others feels good, and to value what he has. He'll also have a new bike. :)
A LeapFrog computer so he can be ahead or right on the mark when he starts school,if he hasn't already. It is a GREAT way to help him expand his vocabulary & intelligence. I have a 4 year old son, and if it were me, I would go for any kind of gift that requires a little imagination, 4 year olds have huge imaginations, art kits, spy kits, cowboy gear, anything they can pretend with and stimulate their imagination and creativity...:)
When my son was young we always bought him a bunch of educational workbooks and activities to help prepare him for school. He would sit and work on them for hours at a time and I think he preferred them to the toys we bought.
At 4, some good workbooks would be for basic writing and the alphabet, penmanship, some easy reading words or flash cards, coloring activities to recognize the letters and numbers, if in the US, a states coloring book or puzzles to recognize the states and etc..
It's amazing how much they learn at that age. If you already do the education workbooks, try some craft projects like modeling clay that you can bake up later, or spooling, things to keep him busy and entertained.
I've also seen some of the interactive games that hook up to your tv...one I saw had a music keyboard that the child jumps on to play music on the tv, another was a mat on the floor that the child would learn different dances from by following the footstep map on the mat...pretty cool.
How about an art box filled with different art supplies? Crayons, glue, paint, paper, stickers, etc...? You can even buy a tub or some kind of container and personalize it with his name, and fill it with the supplies. Here are some fun websites your son might enjoy... nothing to do with a birthday present, but my 4 year old loves these...
