15 month antediluvian gag?
Could it have anything to do near an ear infection (you know that itchy feeling in the spinal column of your throat?)?
He has a runny nose but otherwise he is fine.

My daughter did the same item. I just told her not to do that anymore and she eventually stopped. I never spoke to my doctor about it as it be only a quick phase and I be told it was a phase by others. So I don't have medical suggestion on this one as to if it could be related to a medical problem. My daughter's 2 1/2 and she was doing this (and still does sometimes) when she was contained by trouble or because she knew it made me mad. So, when she tries it, I speak about her "If you throw up, you'll go to time out". It may sound a bit mean, but I got tired of cleaning up vomit everywhere, and I if truth be told only had to put her within time out once for it. She learned from that.
If he has a runny snout, some of his mucus could be dripping down his throat, making it sore. That's happened to me, I thought it was strep and the doc told me it be just the nasal fluids making it sore. I'd take him to a doctor if he doesn't stop soon, they should be capable of help him. He has academic, and maybe by accident that he can control something. My 21 year antediluvian used to think it was fun to throw up, until I have to give her Ipecac once. Just let him know that he have to keep his food down to grow big and strong. And remember, he is a boy...if it's gross and makes sounds, it's for him. He will stop gag himself, but I'm afraid that he won't grow out of loving gross noises.
My son did that a lot and it drove me crazy. I know how your sensation. My son was teething really badly when he started. Eventually he stopped doing it. I say-so try to distract him with doing something else when he starts to do it. He's probably just entertaining himself - seriously. I've see my own and other kids spontaneously vomit just because they can. Don't worry give or take a few it, it'll pass when he tires of it.
Noah's done this a couple of times and has if truth be told thrown up...yuck! He's doing it because he is teething (6 big teeth at once). I try to distract him w/ plastic golf balls. I put one in respectively hand. Good luck. Pull boxing gloves over his hands.
He is basically playing, all babies do this at some point :) Calling the pediatrician is always a apt idea when you're not sure. The doctor can clear this up for you better than people on here.

his throat may be bothering him hun. go to doctor bring him to the doctor even though u think its not that big it might be like if yall own cats or dogs around the house he might have ate a hair or a bug or something
it might be a ear infection but he could anyone doing that for attention try to not pay it no mine for about five minutes but keep watch on him carefully why he is doing that but let him see you watching him I would def. settle to his doctor about this, but there probably is something within his throat thats bothering him.
Try giving him a popsicle, or they make cold and sore throat lollipops
